USE THESE INSTRUCTIONS TO LOG IN – IGNORE EMAIL YOU RECEIVED
To log on to the patient portal:
Patient Portal- Click on “Patient Portal”
- Continue to Portal
- Login (username and password at bottom of this sheet)
To Compose a Message:
- Go to the envelope in the upper right corner
- Compose message (see below to see who to send it to)
- Must put a “subject” in to be able to send
If you are having trouble logging in or difficulty navigating with the patient portal – please call 315-451-5400 – option 1 and extension 4702
For Prescription Refills:
Compose message to “Prescription Refills” – which will populate as Andrea Prescription Refills (do not use the refill/prescription tab)
In the message – please list the following
- Name of the medication
- MG and how many a day you take
- Specific pharmacy medication needs to be sent to
- You will receive a message back indicating you request was received
Please allow 5 days for refills to be filled.
For Appointments:
Compose message to “Ashley Appointment Changes S”
You are able to confirm, cancel or change an appointment this way
In the message – please list the following
- Date of appointment you are referring to
- If canceling – reason why
- If you want to reschedule – the best date and time that works to be rescheduled to
You will either receive a call or a message back within 48 hours.
